Vacancy Location: Covering a case load in the North East, with some hybrid working
Salary / Rate of Pay; FTE £27,000
Contract type; ; Full Time. Permanent completing 37 hours per week, there is the possibility of completing overtime/sleeps if available.
Essential Criteria:
- A degree in Psychology, Occupational Therapy, Nursing, NVQ Level 3 in Health and Social Care or related field or, equivalent work experience
- Proficiency in performing personal care tasks, ensuring the well-being and comfort of the individual under your care.
- A naturally positive attitude that fosters a supportive and uplifting environment for those you work with.
- Proven ability to inspire and motivate the individual you support, empowering them to pursue their goals and aspirations with determination and enthusiasm.
- Proven ability to lead and or manage a team.
- Full UK driving licence and access to a vehicle.
We are currently recruiting 2 Case Managers to join our team of operational managers;
Role 1 – Experience supporting individuals with complex nursing needs, either within healthcare settings or in their own homes, is essential for this role and your caseload. Due to the personal care needs involved, this role is open to female applicants only, in accordance with Schedule 9, Part 1 of the Equality Act 2010.
Shift pattern(s) available; The typical working pattern will be 09:00–17:00, Monday to Friday. However, as the service operates 24/7. As part of our contingency planning, there will be occasions when session delivery outside of these core hours is required

Role Specifics; The aim of this role is to oversee support packages for people with nursing requirements. Experience in providing support to individuals with nursing needs such as; PEG, Ventilator, Conven, Cough Assist, Oral Suctioning, etc. You will be following nursing care plans and showcasing an understanding of their unique needs. Our goal is to enable the people we support to become more independent and live within their own home or university. A huge focus of the job role will be on the quality of support, supportive line management of staff, open and clear communication with the people we support and their family. You will lead and motivate a support team to achieve any personal specific goals. To do this, you will collaborate with our Clinical Nurse Educator to develop goals and interventions, review care plans and review the quality of support provision.
Role 2 – Experience in providing support to individuals with Brain Injuries, following Rehabilitation care plans and showcasing an understanding of their unique needs and challenges, is essential for this role and your caseload. Due to the personal care needs involved, this role is open to male applicants only, in accordance with Schedule 9, Part 1 of the Equality Act 2010.
Shift pattern(s) available; The total working hours (37 per week) are divided between office-based work, home working, and session delivery. The session delivery element of your role will incorporate 18 hours of support sessions per week, across your caseload.
This is the perfect role for someone who would like to expand their ABI and Rehabilitation experience whilst gaining or expanding their current management experience. We are looking for a highly resilient Case Manager who can navigate complex situations with confidence and professionalism. You will have the ability to manage conflict effectively, approaching challenges with a solution-focused mindset. Your strong communication skills will enable you to address difficult conversations positively, ensuring that all parties feel heard and valued while upholding the best outcomes for service users. Encompassed within both roles are the additional tasks:-
Support Provision
- Your working hours will be a combination of delivering support in the home of the people we support, and office-based work on a rota basis over seven days.
- To deliver high quality, safe and person-centered support.
- To be part of a contingency plan covering absences and holiday cover on the package you manage.
- To safeguard the person we support at all times and always act in their best interests.
- To develop a rapport and professional supportive relationship with the person we support and their next of kin.
- To identify achievable support and personal goals with the person we support.
- Manage and lead Support Workers to follow and implement care plans.
- To agree a set rota of support sessions with Commissioners, People we support and Support Workers, ensuring that funded support hours are delivered and not exceeded.
- To be part of an on-call rota, providing advice, guidance, and support to the service outside of regular working hours.
- To ensure the Service Manager and Registered Operations Manager are kept up to date on support delivery, achievements, concerns or issues within the support package.
Line Management
- To act as line manager to Support Workers.
- To maintain rota management for the 24/7 case and support team.
- To complete regular supervisions and observations of the staff being line managed.
- To provide support, guidance and training to staff being line managed, as required.
- To act as a role model and lead by example.
- To communicate clearly and effectively with staff teams at all times.
- To work alongside our Recruitment team to recruit Support Workers based on individual needs and preferences.
- To brief and train Support Workers in individuals’ specific needs.
